Membership Instructions

Here is what you need to know about all forms, applications, and waivers. We want to simplify the process and clear up any confusion.

The forms that must be filled out and returned are:

  1. Membership Application (MUST be accompanied with a copy of Driver’s License or State issued ID.)
  2. Agreement-Waiver Form This must be signed and returned by EVERY rider, regardless of age. We only need pages 4. Please do not send every page, keep that for your reference.
  3. Waiver and Release by Legal Guardian or Parent – This must be signed and returned for any rider 17 years old or younger. They will not be able to ride without this form. (Don’t forget even minors must sign Agreement-Waiver page 4.)
  4. Both husband and wife need to sign individual waiver forms and need to fill out individual applications.
  5. Any spouse that wants to come to the park but does not want to ride can get a nonmember card at no charge.
  6. Waiver is valid for 1 year of date of signature.
